EmploymentAffirmative Action / Equal Opportunity EmploymentOutreach/Mobile Crisis Clinician Outreach/Mobile Crisis ClinicianResponsibilities: Serves as the primary clinician for clients who require community outreach and treatment. Provides outreach, developing initial and on-going recovery plans, implementation of the treatment plan, coordination of client needs and services, and emergency assessment/crisis intervention. Utilizes all required assessment tools which measure the client’s functionality, needs, preferences, and appropriateness of services. Works with clients in urgent or emergent situations towards a goal of residing in the least intensive level of care necessary and avoiding unnecessary inpatient hospitalization. Provides on-site and mobile crisis evaluations for individuals with psychological and/or substance abuse disorders facilitating appropriate follow-up for community services and/or hospitalization. Meets all applicable and current requirements for service, utilization review, and reporting procedures per the South Central Crisis Service’s Policy and Procedure Manual. Oversees, with the program manager, admissions/discharges of the SCCS respite bed. Collects, reviews, enters into the DPAS system, all crisis services performed by the agency and ensures data accuracy. Demonstrates an ability to develop a basic level of competency for providing services to clients with co-occurring disorders. Works collaboratively with treatment teams to engage difficult to serve clients, utilizing psychiatric medical consultants as needed. Fosters collaboration with outside treatment providers and among agency departments. Acts as liaison to local emergency departments. Provides backup for the Jail Diversion Coordinator. Writes Emergency Certificate for Examination/Psychologist Emergency Evaluation Certificate as appropriate in the event the client needs transport to the hospital emergency department. Demonstrates the ability to understand the Medicaid Rehab Option (MRO) process, the delivery service required for Targeted Case Management (TCM), and the Integrated Dual Diagnosis Treatment (IDDT) principles utilizing Motivational Interviewing. Monitors and analyzes utilization review and productivity of caseload and makes adjustments as needed. Maintains and updates, on paper or electronically, client case records with required progress notes documenting progress toward service goals and objectives. Enters daily service activities into the data base and insure its accuracy. Provides timely documentation and data to Agency departments, DMHAS, or other sources as requested. Participates in program planning and development. Completes duties outlined in position’s performance standards within compliance ratings. Attends training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing. Adheres to Agency policies and procedures, including ongoing compliance with accreditation standards. Attends departmental and agency meetings as required. Performs other duties as assigned.
Hours: Monday through Friday: 8:30AM to 4:30PM. Requirements: Master’s Degree in Social Work, or Ph.D. in Clinical Psychology. Clinical experience in a psychiatric or substance abuse setting, applicable license required. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Agency’s auto insurance plan. Must obtain Emergency Certificate. To apply for an open position:
