Case Management
A coordinating and problem solving program designed to assure the continuity of care and successful navigation of the federal and state system to obtain needed services.
Case managers provide support to Individuals based upon the presenting needs and preferences of the client. Depending on the intensity, the frequency of support and service can range from meeting daily to meeting monthly.
Assistance provided with:
Linking to housing resources
Admission into programs
Obtaining and maintaining financial benefits
Transportation issues
Health and behavioral health services
Linking with community resources
Planning, monitoring progress, and reviewing goals
Advocacy and client empowerment
Activities of daily living/skill building
Assertive Outreach/Community Treatment
Provides engagement and treatment within the community to clients who are unable or unwilling to come in for treatment, but still require services. Works with private and community providers as well as police, hospitals, churches, social service organizations, etc. to assess an individual’s needs to remain independent in the community.
